Sliding off running shoes

Every now and then I will notice that patient’s feet will slide laterally off the side of athletic shoes when in stance. Meaning the lateral upper will be very snug at the point of the 5th metatarsal head, part of the lateral border of the foot will rest off the midsole and the medial side off the upper is loose. I have noticed this in neutral style shoes, no medial wedging to midsole and with no orthotics
